Geoffrey Chaucer is considered the father of English literature. He was born in 1343 in London and received an excellent education. His masterpiece, The Canterbury Tales, is a narrative poem written in verse containing stories told by a group of pilgrims traveling to Canterbury Cathedral. It provides insight into the English society of Chaucer's time through its detailed descriptions and variety of characters. The stories range from religious to humorous, with the structure involving an introduction of the pilgrims, their individual tales on the journey, and a contest to determine who tells the best story.
This chapter discusses the importance of taking a developmental approach to the history and physical examination in paediatrics. Key points include:
- Children are not miniature adults and development impacts their care needs. Understanding child development is important.
- Flexibility is key when assessing children as their responses will guide the interaction. Safety is also paramount.
- The history and exam should be tailored based on a child's age and development. For infants, attachment to caregivers is important to respect, and the birth history and family's coping ability are relevant.
- Toddlers are increasingly independent but still dependent on caregivers. Their mobility impacts the exam and history should include social/behavioral information.
- For school
This document provides information about porphyria, a group of rare inherited or acquired disorders affecting enzymes involved in heme production. It discusses how legends of vampires and werewolves may have originated from symptoms of severe porphyria cases. The types of porphyria that affect the nervous system or skin are described. Notable historical figures believed to have had porphyria are mentioned. The mechanism of heme synthesis and how deficiencies cause porphyria are explained. The main types of porphyria are outlined along with their associated symptoms. Diagnosis involves blood, urine, and stool tests, while treatment depends on the type but may include medications, injections, phlebotomies, or avoiding triggers.
Male hypogonadism is a condition where the body does not produce enough of the male sex hormone testosterone. There are two types: primary, where the problem originates in the testicles, and secondary, where it is caused by issues with the hypothalamus or pituitary gland in the brain. Untreated hypogonadism can lead to complications like abnormal genitalia, infertility, erectile dysfunction, and osteoporosis. Diagnosis involves hormone testing, semen analysis, and imaging, while treatment is testosterone replacement therapy.
Anemia can be seen in the emergency department both as a primary pathological process or secondary to both medical and surgical diseases. Moreover, acute anemia can occur in children who have been otherwise healthy, who have systemic disease, or who have known hematologic disorders. Anemia may indicate a disorder with a single hematopoietic cell line (eg, red blood cells) or may be associated with changes in multiple cell lines indicative of bone marrow involvement, immunologic disease, peripheral destruction of erythrocytes, or sequestration of cells. Independent of the etiology, prompt diagnosis is predicated on understanding the classifications of anemia, the associated presenting symptoms, and the proper ordering and interpretation of laboratory studies. This article will discuss the evaluation, proper classification, differential diagnosis, and initial management of acute anemia using cases representative of those that might be seen in the pediatric emergency department.
G6PD deficiency is a defect in the G6PD enzyme, which provides protection against oxidative stress in red blood cells. It is an X-linked inherited condition, though female heterozygotes have some protection against malaria. Those with G6PD deficiency experience hemolytic anemia during times of oxidative stress caused by factors like infections, medications, or foods like fava beans. The deficiency results in inadequate levels of NADPH and glutathione, leaving red blood cells vulnerable to damage and hemolysis. Symptoms of the acute hemolytic anemia appear 24-48 hours after exposure to the triggering agent. Laboratory tests show signs of hemolysis and low G6PD enzyme activity. There is no cure or treatment other than
Iron deficiency anemia is a common type of microcytic anemia caused by low levels of iron available to produce hemoglobin. It affects people with high iron demands like pregnant women, young children, and menstruating women. Symptoms include fatigue, weakness, and shortness of breath. Treatment involves oral iron supplementation and dietary changes to restore iron stores. Prevention focuses on adequate iron intake through diet or supplements in at-risk groups.

Phenylketonuria (PKU) is an inherited disorder caused by a deficiency of the enzyme phenylalanine hydroxylase, resulting in an inability to metabolize the essential amino acid phenylalanine. Left untreated, it can cause severe intellectual disability. It is diagnosed through newborn screening and treated with a phenylalanine-restricted diet. For pregnant women with PKU, careful control of phenylalanine levels through diet is needed to prevent complications in the fetus like intellectual disability and birth defects. The case study describes a woman with mild PKU who was able to deliver a healthy infant by maintaining her phenylalanine-restricted diet during pregnancy.
Glycogen storage disease (GSD) occurs due to defects in enzymes involved in glycogen synthesis or breakdown, leading to excess glycogen storage. There are 11 known types classified by the affected enzyme and tissue. Symptoms vary by type but can include hypoglycemia, liver and muscle involvement, and exercise intolerance. Treatment depends on type and may include dietary changes, enzyme replacement therapy, or liver transplantation. Prenatal testing is available for some types.

Anaemia is defined as a low level of haemoglobin in the blood. It diminishes the blood's ability to carry oxygen. Normal haemoglobin levels vary based on age, sex, and life stage. When haemoglobin levels are subnormal, the body compensates by increasing oxygen release and blood flow. Common symptoms of anaemia include tiredness, weakness, and fatigue. Anaemia is classified and diagnosed based on red blood cell size, shape, and laboratory findings. Causes include iron deficiency, vitamin deficiencies, genetic disorders, and underlying chronic diseases.
This document discusses diseases of the haemopoietic system. It begins by describing blood cells and bone marrow haemopoiesis. The main types of diseases are then outlined as anaemias, haematological malignancies, and bleeding disorders. Iron deficiency anaemia is discussed in detail as the most common type of anaemia. It is caused by blood loss, increased demands, malabsorption or dietary deficiencies. Clinical features include pallor, spoon-shaped nails, and cognitive effects in children. Laboratory tests can identify microcytic hypochromic blood cells characteristic of iron deficiency.
Clinical features and diagnosis of malabsorptionFarshad Mirzavi
The document provides an overview of malabsorption, including its causes, mechanisms, diagnosis, and laboratory tests used for evaluation. It discusses the three key steps required for normal nutrient absorption and lists factors that may damage the intestine and cause malabsorption. Common tests described include stool tests to measure fat, blood tests of vitamin/mineral levels, breath tests for lactose intolerance, imaging to detect structural abnormalities, and biopsy for abnormal cells. Carbohydrate and fat malabsorption are evaluated using tests like D-xylose absorption, lactose tolerance, breath hydrogen, and fecal fat determination. Specific disorders are investigated using antibodies for celiac disease and the Schilling test for pernicious anemia.
The document summarizes the key events and monarchs of the English Restoration period from 1660-1714. During this time, Charles II was restored to the monarchy after exile in France. His brother James II succeeded him but was overthrown in the bloodless Glorious Revolution of 1688. William and Mary then took the throne, followed by Queen Anne, who united Scotland and England during her reign. The Restoration period saw the founding of the Royal Society, and influential philosophers like Locke, Newton, and Hobbes emerged.
The English Renaissance spanned from 1485 to 1625 and was inspired by ancient Greek and Roman arts and scholarship. This period saw major figures like Dante, Petrarch, da Vinci, Luther, and Erasmus. In England, the Tudor dynasty ruled from 1485 to 1603 with monarchs like Henry VIII, Mary I, and Elizabeth I. Elizabeth I was one of England's ablest monarchs and presided over a flourishing of English literature and drama including works by Shakespeare, Spenser, Marlowe, and Raleigh. During this time, plays were categorized as either tragedies involving a hero's downfall or comedies with a humorous situation resolved happily.

This document discusses ballot measures and initiated measures in South Dakota and other states. It provides data on the number of states that allow initiatives, referendums, and constitutional amendments to be placed on ballots, and the signature requirements to do so in various states. It also shares statistics on the number of ballot measures nationwide in recent elections. The document advocates opposing Initiated Measure 23 in South Dakota, citing concerns that it is vaguely worded and could force workers to pay fees to unions even if they do not want to join. Several arguments opposing the measure are presented and polling data shows most voters oppose it after hearing descriptions of what it does.
A poll of 1038 likely Democratic primary voters in Massachusetts' 8th Congressional District found:
- Stephen Lynch has a 46% favorable rating compared to Robbie Goldstein's 36% favorable rating, with Lynch leading 39-32 if the primary was held.
- Among undecided voters, support for a progressive Democrat was higher than for a moderate, and support for a pro-choice candidate was much higher than a pro-life candidate.
- A majority of undecided voters would also support a candidate who backs Medicare for All over one who opposes it.
- The poll had a 3.04 point margin of error and was conducted August 8-9 via phone and text with respondents' gender and age demographics
The document summarizes the results of a poll of registered voters in Hawaii. It finds that 48% feel things in Hawaii are going in the wrong direction, while 38% say right direction. Approval ratings for political figures like Governor Abercrombie, Senator Schatz, and Congresswoman Hanabusa varied from 31-58%. On key issues, support was mixed for legalizing medical marijuana dispensaries (45% yes, 47% no) but less for recreational use (33% yes, 59% no). Two-thirds supported raising the minimum wage, though views were divided on the economic impact. A majority approved of President Obama's job performance.

This survey aims to better understand the reality of how often pregnancies result from rape by gathering data from Minnesota. Currently, the only nationwide statistic estimates the rate of pregnancy from rape is 5% annually, but this number may vary significantly between states. The survey contains triggering questions for sexual assault survivors and provides an opt-out option. It asks individual survivors and representatives of organizations that serve survivors about their demographics, experiences with pregnancy resulting from sexual assault, and interactions with the criminal justice system. The goal is to learn more about how Minnesota women are affected by pregnancies from rape.
Voices from the Newly Enrolled and Still Uninsured: A Survey about the Afford...Enroll America
1. The document summarizes findings from two surveys - one of newly insured individuals and one of those who remained uninsured after the first open enrollment period under the Affordable Care Act.
2. For the newly insured, the top reasons for enrolling were that it's the law, to access healthcare, and to avoid fines. Younger adults were more motivated by fines while older adults wanted healthcare access.
3. The newly insured had higher levels of knowledge about the ACA compared to the uninsured. However, both groups cited costs as a key barrier to enrollment.

Burning Glass Research on Planned ParenthoodBurningGlass
Burning Glass Consulting conducted research with key women voters about their attitudes toward Planned Parenthood. They found that most women are not familiar with recent videos about Planned Parenthood and do not closely follow the related scandal. Planned Parenthood has a strong brand among many women due to their positive experiences there. However, the videos raised concerns about whether Planned Parenthood profits from fetal tissue and has patient consent. Most women support a full investigation into Planned Parenthood's practices before defunding but not an immediate government shutdown over the issue.
The document discusses various perspectives on the topic of abortion. It provides definitions of abortion and outlines typical anti-abortion and pro-choice arguments such as a woman's right to choose versus an embryo's right to life. Survey results are also presented, finding that respondents were split on whether and when abortion should be legal. The document concludes that views on abortion are complex, with most believing it should be legal under some circumstances but with limitations.

Three surveys conducted by the Kaiser Family Foundation in February 2013 found:
1) Americans remain split in their views of the Affordable Care Act, with slightly more holding unfavorable than favorable opinions.
2) Hispanics and blacks were more likely than other groups to say they worry about being victims of gun violence and to personally know someone affected by gun violence.
3) While most Americans favor requiring equivalent mental health benefits, only about a third correctly understood current law as requiring parity between mental health and other medical benefits.

The document provides an overview of public opinion on health care issues in the United States. It finds that while Americans have less confidence in the US medical system and many believe it needs major changes or a complete rebuild, the vast majority are still satisfied with their own medical care and coverage. It also finds ongoing partisan divides and confusion over the Affordable Care Act, with opposition consistently outnumbering support. Most Americans feel they lack information to understand upcoming changes and the new health insurance exchanges.
